Heather Gay has been candid about her weight loss.

The Real Housewives of Salt Lake City star first revealed that she was using Ozempic for weight loss in November 2023. She has since shed nearly 30 lbs. after struggling with body image issues all of her life.

Over the years, the Bravo cast member has opened up about different aspects of her health journey, from her castmates’ reactions to quitting Ozempic. But she has been most surprised about how her relationships have changed since she’s slimmed down.

“I never realized how much my weight affected the way people treated me,” she said during an appearance on ABC News in December 2024. “But I have loved all the versions of myself and I feel like body positivity wasn’t a lie for me. It changed my life. It allowed me to be on TV, it gave me this chance.”

She has struggled with her weight for her entire life

During a March 2024 appearance on the ABC Audio podcast Pop Culture Moms, Gay shared that her body image journey has been lifelong.

“There’s never been a time in my life that my weight hasn’t been an issue,” she said. “And I guess there’s a lot of sadness, because my weight’s been up, my weight’s been down, and I’m, of course, keenly aware of how the world treats me when my weight is down versus when my weight is up.”

In November 2023, Gay revealed to PEOPLE that she was using Ozempic — an FDA-approved prescription medication for people with type 2 diabetes — to help her lose weight.

“I’ve been on it for a long time, but hadn’t really seen much results. And I haven’t had massive results, but enough for people to notice, which is great,” she said.

Gay later told ABC News in March 2024 that the medication had helped her lose nearly 30 lbs.

At the time, she said she felt “pressure” to take the drug, since she “didn’t wanna show up at another party and see all of my friends 20 pounds thinner and just be resentful.”

Nine months later in December 2024, Gay revealed that she was no longer taking Ozempic, but she “will if I need to.”

She said she was treated differently when she lost weight

Gay has criticized the way that people have treated her since losing weight. While speaking with ABC News in March 2024, Gay said that Ozempic helped her “feel seen for the first time.”

“Even after being on television, writing a New York Times bestselling book, for the first time, I was being valued — by my castmates, by the public — in a way that I had never been valued before,” she said. “And that felt, to me, sad.”

At the time, Gay felt as if she’d “given up the banner” of being a “relatable” figure for her body image journey.

“It’s disappointing and sad to know that body positivity was all a big lie,” she added. “Because it’s better to not be overweight.”

The reality star later clarified to ABC News in December 2024 that “body positivity wasn’t a lie for me.”

Gay had more confidence before losing weight

Although Gay has “loved all the versions” of herself, she told ABC News in December 2024 that she felt more confident before using the drug.

“I look at those pictures of myself and those episodes and think, I love that girl,” she said about her introduction on Real Housewives of Salt Lake City. “That girl was braver and stronger and had more confidence being overweight than I did after taking medication.”

During the interview, Gay also addressed the cover of her 2024 memoir Good Time Girl, where she poses with her shirt lifted.

“I’ve apologized for my body my entire life … and I’d rather just have body neutrality,” she said. “I love myself heavy. I love myself now. It’s a journey and that changed the way I saw who I was.”

She’s open with her daughters about her health journey

Gay shares daughters Georgia, Ashley and Annabelle with ex-husband Bill Gay. When it comes to discussing body image with them, Gay told ABC News in March 2024 that she wants them to “feel empowered,” but it can be “tough” to have those conversations.

“Because I don’t want to lie to them and say, ‘It doesn’t matter. What matters is on the inside,’ ” she said. “But it does matter. I don’t know why that’s the way it is in the world, but that has been my experience.”

Gay added, “I have beautiful, vibrant, empowered daughters and I would hate for them to think that their value is limited to their dress size.”

Gay shut down claims that she’s changed since losing weight

While speaking with E! News in September 2024, Gay fought back against critics who shamed her for using Ozempic.

“I’m still the same. I feel the same. I feel like I liked myself then, I like myself now. It’s just Ozempic became available and it worked for me,” she said.

At The Real Housewives of Salt Lake City season 5 reunion in January 2025, Gay addressed the allegations that she’s become a “mean girl” since losing weight.

“I guess when I was heavier, I was more of an underdog so it was okay when I stuck up for myself,” the reality star said, per E! News. “I had to fight a lot harder battles and I was a lot meaner and I brought a lot more anger and rage than this season for sure.”

However, Gay acknowledged that she has changed, but not in a negative way.

“Because I am happy and I am proud of myself and I don’t want that to make me meaner,” she said. “I am different, but it has nothing to do with the fact that I took medication to make me lose weight.”
